In order to make the day of your evaluation run as smoothly as possible, I have put our intake forms online for you to fill out and turn in prior to your appointment. This allows me to minimize the amount of time spent filling out this form during your appointment and allows more time for skilled evaluation and treatment.

Rates are by appointment. We also offer “packaged” rates for 6 and 8 appointments. Please contact us to learn more.

Each appointment runs 1 hour in length and occur at a location predetermined between the individual and PT (home, office, other…)
